Hello again!
Below you will find a resource that you may find useful to keep motivated and to track your health routine. You can use these documents to track your daily food / calorie intake, physical activity, blood pressure, weight, water intake, step count, SMART goals, Heart Rate, etc. Then, as you continue on your journey to better health, you will be able to every step of the journey that got you there.

Download, print (instructions below), and fill out these documents and as you progress with your health routine, you can keep a track of all your health changes to help you understand what works, and what doesn't!
Show your friends / family / healthcare practitioners and you can show them some solid evidence of all the positive steps you're making to bring about the positive health changes.
If you are coming along to the Movement Medicine exercise classes, you can either send them to myself or bring them to the sessions to show your progress!
Make sure you fill them out truthfully though! ;)

PDF: Cover Sheet & 2 x Day by Day Intake & Physical Activity Diary (print this once):
1 Print = Cover sheet & 2 days diary = 1 piece of paper with double sided printing
How to print: Download & open with acrobat / browser > choose double sided printing > print x 1 (cover sheet) > hit print!
If it does not fit in the margins when you print, choose fit / reduce the page sizing % in the printing options
PDF: 4 x Day by Day Intake & Physical Activity Diary (print this multiple times):
1 Print = 4 days diary = 1 piece of paper with double sided printing
How to print: Download & open with acrobat / browser > choose double sided printing > print however many are needed > hit print!
If it does not fit in the margins when you print, choose fit / reduce the page sizing % in the printing options
